1. The Interview includes: the chief complaint, history of present illness, past medical history, review of systems, family history, social and environmental history, and medications and allergies
2. The Physical Exam includes: the level of consciousness, personal hygiene, major systems, posture and motor activity, height and weight, dysmorphic observations, and vital signs.
3. The Neurological Exam includes: mental state evaluation, cranial nerves evaluation, motor system evaluation, reflex testing, sensory system evaluation, equilibrium evaluation
4. Laboratory Testing includes: body tissue/fluid (e.g.blood, etc.), imaging studies (e.g. CT, MRI, etc).
